Community Health Services receives kudos

Community Health Services, which has a Fostoria location at 502 Van Buren St., has received four Community Health Quality badges by the Health Resources and Services Administration for 2025.Community Health Quality Recognition badges are awarded annually to health centers across the country, recognizing those organizations that have achieved quality improvements in the following areas: access, clinical quality, health outcomes and health information technology.The badges are awarded based on the latest Uniform Data System reporting year. The UDS is a standardized reporting system used to collect consistent information about health centers' performance and operations. It provides a structured way to gather and analyze data on patient characteristics, services provided, clinical processes, health outcomes, staffing, costs and revenues.The collected data help assess the impact and performance of the community health centers, identify trends and improve the quality of care.Community Health Services was recognized in the following categories:National Quality Leader in Heart Health Badge — recognizes Community Health Services as a top-performing health center in the nation in cardiovascular care;High-Value Care Badge — recognizes CHS for meeting or exceeding The Quality Score, which is based on health centers' Clinical Quality Measures.Health Center Quality Leadership Bronze Badge -- recognizes the organization's performance across key clinical quality measures. CHS was ranked in the top 21-30%. Advancing Health Information Technology Badge — recognizes innovation in using data and digital tools. This includes offering telehealth services, exchanging clinical information online with key providers' health care settings and engaging patients through Health Information Technology.Community Health Services operates federally qualified health centers in Sandusky, Huron, Wood, Hancock and Henry counties in Ohio, providing medical, dental and behavioral health services to approximately 12,000 people per year.
